I have only seen a "setover" here (that is where the tow was bigger than the chamber and the towboat breaks apart and goes alongside the tow) one time in almost 20 years! Now down river where the chambers are only 600' long... They do double lockages all the time... When I worked at Strong there would be kids brought into the emergency room for that exact problem. A lot more than you'd think.
