It's called a Kernel Panic, and it's usually the result of human error

Although it never happened to me, Wikipedia accounts of this business model told of instances where a customer would fill out the slip, wait while a clerk looked for the item, and then be told, "Oops, sorry, out of stock." I seem to recall reading in the Buffalo News that how New York state changed taxation of these stores resulted in their demise.
