The Cardinals had stalled out after Palmer was sacked by The Lorax on third and long; only to have the drive extended because Hughes thought it was a good idea to throw Palmer to the ground by his head, ripping his helmet off in the process.

None of the other things you mention need to occur if Hughes isn't a bonehead.

Also, the Hughes situation was different because the Bills already had completed the stop when he was penalized
