Some think that human error issue cost Toyota about a billion dollars. Of course we had to save GM from going bankrupt (although they actually ended up doing that anyway) but hey - it's all good. Nanker mentioned Toyota - I have two, and in both it's pretty easy to catch the edge of the brake pedal when you hit the gas, or conversely have your foot slip off the brake and hit the gas pedal. And if you're an dumb, panicky idiot who reacts to having your foot on the wrong pedal by stomping your foot as hard as you can, that can cause an accident.
