I think prescribing off-label is a gray area.  I take it on a drug by drug basis.  Again, a discussion of risks and benefits is paramount.

No personal experience with compassionate use.  Don't see a problem with it in theory.

Understandable. I'm an endocrinologist - experienced compassionate use for the first time in 2015 and it worked out well for the patient. The drug's now approved, but part of the deal was the patient had to be referred to NIH to enter their data into the final stage of the trial. I've had very few patients ask me about pot, however it's my opinion 99% of the available research on it is biased in one way or another. It needs to be decriminalized or rescheduled so it can be properly studied.

To be perfectly honest, my first reaction to Trump supporting legalization, is to question my support of it.

It's like when people told me the VW bug was an Idea from Hitler... made me dislike Bumblebee, a little.

I guess marijuana reform  support from the left just evaporated

Some detailed information on how to utilize cannabis to reduce pain management opiate use.

Dr Dustin Sulak is based in Maine but has put a good deal of medical cannabis information on line.  See his healer.com site.

https://www.leafly.com/news/health/using-cannabis-to-reduce-opioid-dependence

Like the reports in the scientific literature, I’ve seen the same positive results firsthand in my medical practices. From a survey of our patients in 2016, of the 542 opioid users who added cannabis:

  • 39% were able to completely stop opioid use
  • 39% used cannabis to reduce their opioid dosage
  • Adding cannabis reduced pain by more than 40% in nearly half the patients and improved function in 80%
  • In 87% of patients, it improved quality of life

GW Pharmaceuticals is taking the FDA route with Epidiolex.  They have successfully completed phase 1-3 trials and gotten unanimous approval from an FDA advisory panel.  If I recall, the FDA will make a decision on or before 6/27/18.  After that, the DEA will decide if they will allow Epidiolex in spite of the schedule 1 designation for whole cannabis - a dual schedule.

Yesterday the FDA approved this cannabis based medicine for 2 forms of child onset epilepsy.  The DEA is yet to rule

https://www.cbsnews.com/news/fda-approves-epidiolex-first-marijuana-based-drug-for-seizures/

U.S. health regulators on Monday approved the first prescription drug made from marijuana, a milestone that could spur more research into a drug that remains illegal under federal law, despite growing legalization for recreational and medical use.

The Food and Drug Administration (FDA) approved the medication, called Epidiolex, to treat two rare forms of epilepsy that begin in childhood. But it's not quite medical marijuana.

 "Doctor" Stuart Titus has a degree in economics from Rollins College (ranked just slightly lower than scholastic powerhouses like Marist or the University of Scranton).  His "medical degree" is actually a Ph.D. in acupuncture from the Open International University For Complimentary Medicine.  And he's being investigated for pump-and-dump medical marijuana penny stock and real-estate Ponzi schemes.  He's actually less qualified to talk about medicine than you are.  But hey...he suckered in Andre Reed, so...credibility!

I'll say it again: the biggest impediment to legalizing marijuana is the complete dopiness of the people promoting it.
