It's such a major, oft-engaged muscle, you'd think it'd be among the more sturdy. Could it have been an exertion issue? Did he just bend funny?

It can be a combination of things....it could be an inner muscle tear or it can be a tendon tear. He was caught in a scum which could have cause his leg to do things unintended or the strain and force of all that contact caused the muscle to tear.
