Yes sadly not all patients respond to Remicade/anti-TNF drugs. Your sister made a brave choice and one that a lot of my patients who have not responded to medical therapy eventually wish they had made years earlier. Fingers crossed for her that she does well!

Gastroenterologist here. Remicade side effects are pretty minimal in day to day life. Bigger risk are big picture things like infections, lymphoma, etc. As he presented with penetrating disease/needed surgery, he's probably on both Imuran and Remicade.
